A monitoring inspection was conducted today. While at the facility to finalize a previous inspection, it was determined that one group of children was out of ratio. See violation notice for non-compliance.

Standard #: 8VAC20-780-350-B-4 Description: Based on observation, the facility did not maintain a ratio of 1 staff to 10 children for a group of children aged 3 years old to school age eligible.
Evidence:
1. At arrival, Licensing Inspectors observed 1 staff member with 18 children "practicing graduation" in a portion of Room 3 (per the facility floor plan). The group of children included 2 children that are 4 years old. (Child #1 Date of Birth: 04/24/2018), (Child #2 Date of Birth: 02/02/2018).
2. The same group was additionally observed in Room 5 (per the facility floor plan) having "Show-and Tell", again with 1 staff to 18 children.Plan of Correction: Not available online. Contact Inspector for more information.
