Comments:
An unannounced renewal inspection was conducted on-site June 23, 2022. The director was available during the inspection. There were 34 children present, ranging in ages from 14 months to 8 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, the center did not ensure that each staff submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence: The record of staff #2 (hired 2/8/22) contained documentation of a tuberculosis screening dated 3/26/22.

Plan of Correction: Will ensure all medications refills are received in original containers at all times regardless of any issues.

Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, the center did not ensure that each staff submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence: The record of staff #2 (hired 2/8/22) contained documentation of a tuberculosis screening dated 3/26/22.

Plan of Correction: Ensure staff TB tests are completed per licensing standards guidelines.

Standard #: 8VAC20-780-500-A
Description: Based on observation and interview on June 23, 2022, the center did not ensure that children wash their hands with soap and running water after toileting.
Evidence: Staff was observed changing a diaper. After the child was changed the child was put back on the floor and preceded to play with toys. The child's hands were not washed. Staff stated that wipes are used after a diaper change.

Plan of Correction: Retrained staff on toileting and hand washing procedures.

Standard #: 8VAC20-780-510-I
Description: Based on observation and interview on June 23, 2022, the center did not ensure that prescription medication is maintained in the original, labeled container that was dispensed from a pharmacy.
Evidence: A review of medications indicated a prescription for Albuterol for child #6 expired 5/25/22. Administration stated they accepted a refill of prescription Albuterol for child #6 and put it in the old prescription box. Administration acknowledged they did not receive the original box for the refill.

Plan of Correction: Not available online. Contact Inspector for more information.
