Technical Assistance:
Today we discussed capeting, chemicals and dates of hire.
Additionally, we discussed Childcare Aware and Subsidy.

Comments:
An unannounced onsite monitoring inspection was conducted June 15, 2022. There were 23 children in care ages infant through five years old with seven staff and two administrators. A walking tour of the entire facility inside and outside was completed. Children were observed in teacher and self-directed activities including play activities, restroom routines, transitions, outside play, lunch and nap. Emergency equipment and evacuation drills were reviewed. Violations were observed, appear on the violation notice and were discussed with the administrators during the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview the center failed to ensure that areas are maintained in safe condition.
Evidence:
In the three classrooms where children 2.5 years and younger were cared for the direct egress is blocked.
a. In the infant room in order to exit the egress door one had to open a secured safety gate and step down a step before reaching the door.
b. In the toddler care room there is a secured safety gate that separates the room in half. Once the safety gate was open the egress door was locked by a deadbolt which was engaged during the inspection.
c. In the young two-year-old room the egress door is locked with deadbolt that was engaged during the inspection.
2 - Throughout the facility each classroom has half doors/Dutch doors as entry into the classroom. The top half of the doors in some cases were observed in the open position. However, the top half of the doors were not secured and as such created a place potentially where fingers could become caught resulting in injury.
- Staff 1 and 4 confirmed that the egress to outside was blocked by safety gate and or deadbolt lock and that the top half of the split doors were not secured.

Plan of Correction: - Center management will retrain all staff that in rooms where children 2.5 years of age and younger are cared care for, the route to exit the room to the outside must not be blocked by items, safety gate or deadbolt.
- Deadbolt locks in rooms where children 2.5 years and younger must and will remain unlocked when children are in care. Signage stating that door must remain unlocked during operating hours will be posted.
- All ? doors upper half will be have hook and eye latches installed so that when the upper portion is open it can be secured so that it does not have the ability to swing closed on its own. When the upper ? of the door is in the closed position a sliding latch will be engaged so that door cannot swing open.

Description: Based on observation and record review the center failed to ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director.
Evidence:
1 - There was no documentation present in the record of Staff 2 to demonstrate that she meets program leader qualifications.
a. Staff 2 was responsible for the preschool group of children during the inspection. There were six children present ages four through five years old.
2 Staff 4 reported that staff 2 is working on completing her GED.
3 - There was not documentation on file for staff 3 documenting that she meets program leader qualifications.
a. Staff 3 was responsible for five children ages two to three years old during the inspection.
b. The documented job title for staff 3 is program aid.

Plan of Correction: Going forward center management will ensure that a program leader qualified person is present in grouping of children.
