Comments:
The licensing inspector conducted an unannounced renewal inspection on Tuesday, May 17th, 2022 from 3:50pm to approximately 4:38pm. There were a total of four children in care in the direct care of two staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children including the cafeteria, the lobby, hallways, and bathrooms were inspected. The required postings were reviewed and found to be in compliance. The center is equipped with toys and supplies and items were available to the children. Medication is administered when required, but there are no medications on site at this time. Five children?s records and three staff records were reviewed. Additional information was later submitted to the inspector and was reviewed virtually.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a observations, interviews, and a review of five children's records, the center did not keep at the center a separate record for one child enrolled.

Evidence: 1) During the inspection on 05/17/22, staff interviewed reported the record for Child #5, enrolled on 01/24/22, was not available for review because it was not kept on-site. 2) Child #5 was present during the inspection on 05/17/22.

Plan of Correction: Moving forward, documentation for each child enrolled will be maintained at the center.

Standard #: 8VAC20-780-70
Description: Based on observations and staff interviews on 05/17/2022, the center did not ensure that the required information for each staff was kept at the center.

Evidence: 1) During the inspection, Staff #1 and Staff #2 stated that the name, address and telephone number of a person to be notified in an emergency for each staff was not at the center. 2) Staff #1 and Staff #2 were observed at the center on 05/17/22.

Plan of Correction: The required information to be kept at the center will be maintained on site where staff are working.

Standard #: 8VAC20-780-80-A
Description: Based on observations and interviews, the center did not maintain a written record of daily attendance that documented the departure of each child in care as it occurs.

Evidence: 1) During the inspection on 05/17/2022, the center did not have a written record of daily attendance that documented the arrival and departure of each child in care as it occurred. 2) During interviews, staff reported the center has not kept a written record of daily attendance in months.

Plan of Correction: Staff will be provided with the equipment needed to document the arrival and departure of each child as it occurs.

Standard #: 8VAC20-780-550-D
Description: Based on a review of the center's emergency drill log and interviews, the center did not implement a monthly practice evacuation drill.

Evidence: 1) The licensing inspector observed the emergency drill log for the years 2021 and 2022. An evacuation drill was not documented in April 2022. 2) During interview, a member of management reported an emergency evacuation drill was not conducted in April 2022.

Plan of Correction: Staff will be reminded to conduct and document monthly drills.
