Comments:
An unannounced complaint inspection was conducted on site on 4/15/2022. The inspectors arrived at 9:15 AM and departed at 11:45A.M. Follow up interviews were completed on 5/20/2022. A complaint was received in the licensing office on 4/13/2022. The preponderance of evidence of care, guidance and protection was supported and complaint is valid.

Description: Based on interviews with staff, the center failed to ensure when staff are supervising children, they shall always ensure their care, protection and guidance.

Evidence:
1. On April 8, 2022 child # 1 left the building. Child # 1 was wearing noise cancelling head phones and was in the back hallway that was used for a gathering space in the mornings. Child # 1 left the area and passed the staff # 1?s office. Staff # 1 came out of the office while staff # 4 was rounding the corner of the hallway and saw staff # 1 come out of the office. The child went through the first door and continued to the main entrance door. Staff # 1 then went to follow child # 1. A college employee was entering the building as the child was exiting. The child then proceeded to run approximately 200 yards up two flights of outdoor stairs. A few seconds later another college employee/parent was exiting the building after the staff # 1 and child # 1. Staff # 1 was not able to run after the child due to a medical condition. Staff # 1 continued to yell at the child to stop. The child was not able to hear any one because of the head phones. The two college employees caught up to the child prior to the child entering another building and carried the child back to the center.

Plan of Correction: A new early childhood director has been hired to help ensure that all children and staff are being supervised. New procedures for supervision has been put in place. The hall way will not be used in the morning for gathering. All children will arrive into their own classrooms.
