Comments:
A monitoring inspection was completed today, there were 60 children present with 9 staff supervising.
The physical plant, children's records, staff records, evacuation logs, the playground, emergency preparedness information, menu's, fire inspection, health inspection, injury reports and emergency supplies were reviewed. The children had Maria cookies and milk for morning snack and Chicken Nuggets, peas, oranges and milk for lunch. Areas of non-compliance were cited in the violation notice. Standard #: 8VAC20-780-130-E
Description: Based on a review of children records, the center failed to obtain documentation of additional immunizations once every six months for children under the age of 2 years old.

Evidence:
Child #4 and #5's records do not contain documentation of additional immunizations once every 6 months for children under the age of 2 years old.

Plan of Correction: I will request the record updates from the parents.

Standard #: 8VAC20-780-60-A
Description: Based on a review of children records, the center failed to obtain required documentation for each child enrolled.

Evidence:
Child #1, #2, #3 and #5's records do not contain documentation of persons authorized to pick up the child.

Plan of Correction: I will contact the parents and collect the information for the children's records.

Standard #: 8VAC20-780-245-H
Description: Based on a review of staff records, the center failed to ensure that staff complete annually a minimum of 16 hours of training appropriate to the age of children in care.

Evidence
Staff #1, #2, #3, #4 and #5's records did not have documentation of the required annual training hours for 2021-2022.

Plan of Correction: All teachers will have required hours of training before the start of school in August.

Description: Based on observation, areas of the inside of the center were not maintained in a clean, safe and operable condition.

Evidence:
In the Toddler room, the paint is peeling off the wall under the window frame.

Plan of Correction: I will contact the church/owner regarding the peeling paint.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to keep hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children.

Evidence:
The toddler room, 3 year old room and the 2 year old room had hazardous chemicals in an unlocked closets.

Plan of Correction: All closets were locked during the insepction.

Description: Based on observation, the center failed to ensure that each rest area be equipped with soap, toilet paper and disposable towels or an air dryer within reach of children.

Evidence:
The bathroom in the 2 year old room was not equipped with toilet paper or paper towels.

Plan of Correction: Toilet paper and paper towels where put into the bathroom during inspection.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center failed to ensure that a nonabsorbent surface shall be used for diapering or changing.

Evidence:
The diaper surface in the Toddler room and the 2 year old room had holes and therefore were no longer nonabsorbent.

Plan of Correction: I will purchase new changing pads.

Standard #: 8VAC20-780-570-A
Description: Based on observation, when a child is placed in a high chair, the protective belt shall be fastened securely.

Evidence:
Child was in a high chair, but protective belt was not used.

Plan of Correction: I will remind teachers to use the safety straps when placing a child in an infant seat or high chair.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center had refrigerated bottles that did not contain the date.

Evidence:
Bottles in the refrigerator in the Infant Room and in the Kitchen were labeled with the child's name, but were not dated.

Plan of Correction: I will remind parents to place the date on bottles and also instruct teachers to add the dates if the parents do not.
