Comments:
A renewal inspection was conducted on 09/06/2022. The Inspector reviewed 3 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe and operable condition.
Evidence:
1. In the pre-k room the cabinet above the sink was chipped and the door frames had peeling paint.
2. In the Discovery pre-k room the toilet seat coating was peeling

Plan of Correction: The following corrective action was provided by the program director during the exit interview: A work order will be placed for all maintenance issues in addition to notifying the district manager.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the school-age room there was an unlocked cabinet containing disinfectant spray and an unlocked teacher cabinet containing 3 bottles of disinfectant spray, tub of disinfectant wipes and a bottle of bathroom cleaner.
2. In the Discovery pre-k room there was an air freshener spray in an unlocked cabinet.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The director will be having a meeting with all staff and maintaining chemicals and hazardous substances in a locked area will be discussed.

Standard #: 8VAC20-780-500-B
Description: Based on observation the center did not ensure all requirements when diapering.
Evidence:
In the Discovery pre-k room there were toys and art supplies stored on the changing table.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The director will speak directly with the Discovery pre-k staff and revisit diapering and only using the changing table for diapering and not for other activities.
