Comments:
A monitoring inspection was conducted today, July 6, 2022. There were 60 children present with 13 staff supervising. The physical plant, medications, emergency supplies, menu's, children's records, staff records, the playground, evacuation logs and bottles and cribs were reviewed. Violations:
Standard #: 8VAC20-780-160-C
Description: Based on a review of staff records, the center failed to ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
Staff #2's record does not contain documentation of a current tuberculosis test or screening result.

Plan of Correction: I will have staff member obtain TB test/screen and place results in staff file.

Standard #: 8VAC20-780-60-A
Description: Based on a review of child records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
Child #2 and child #4's records do not contain the phone number for the physician.
Child #4's record does not contain documentation of viewing proof of the child's identity and age.

Plan of Correction: I will request the information from parents and place documentation in child files.

Standard #: 8VAC20-780-240-B
Description: Based on a review of staff records, the center failed to ensure that prior to staff member working alone with children and no later than seven days of the date of assuming job responsibilities, staff member will complete Recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by 63.2-1509 of the Code of Virginia.

Evidence:
Staff #4's record does not contain documentation of the completion of the Recognizing Child Abuse and Neglect Training.

Plan of Correction: I will have staff member locate documentation and if staff member can not locate documentation, staff member will re-take training.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that center shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to. splintered, cracked or otherwise deteriorating wood, chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snap skin.

Evidence:
In the Toddler room, paint is chipped.
In the Sr. Preschool room the green painting is peeling off of the wall and the blue paint is chipped.

Plan of Correction: I will contact the owner regarding the peeling and chipping paint.
