An unannounced monitoring inspection was conducted on 07/07/2022. There were 67 children, ages 1 month- 12 years, 14 staff members providing direct supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services. The children were observed in free choice activities, outdoor time, and lunch time. The inspector discussed the following with the provider: outside area, physical plant, tripping and choking hazards, training, background checks, and children?s records. A total of 5 children?s records, 5 staff records and 1 medication were reviewed. The inspector arrived for the inspection at 9:15 am and departed at 1:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Standard #: 8VAC20-780-130-A Description: Based on record review and discussion with staff, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: The record for child 4 indicated child 4 first attended the center on 07/19/2021. The immunization record available at the time of inspection was dated 10/27/2021.
Plan of Correction: Staff will ensure immunization documentation is obtained before children attend the center beginning 07/08/2022.
Standard #: 8VAC20-780-140-A Description: Based on record review and discussion with staff, the center failed to obtain documentation that each child shall have a physical examination by or under the direction a physician: Before the child?s attendance; or within 30 days after the first day of attendance.
Evidence: The record for child 4 indicated child 4 first attended the center on 07/19/2021. No physical was available for review during the inspection.
Plan of Correction: Staff will ensure physicals are obtained within 30 days of first day of attendance by 07/08/2022
Standard #: 8VAC20-780-160-C Description: Based on record review and discussion with staff, the center failed to ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence: The most recent tuberculosis test/screening for Staff 3 was dated 03/27/2020. There was not a follow-up test/screening for 2022 available for review.
Plan of Correction: Staff have requested Staff 3 obtain test/screening by 7/8/2022.
Standard #: 8VAC20-780-290-A-3 Description: Based on observation and discussion with staff, the center failed to ensure that areas used by children of preschool age or younger, electrical outlets shall have protective covers.
Evidence: One electrical outlet was uncovered in the hallway across from Class 3. This hallway is used by children to enter and exit the center.
Plan of Correction: Outlet was covered at the time of inspection on 07/07/2022.
Standard #: 8VAC20-780-330-B Description: Based on observation and discussion with staff, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
Evidence: There is a metal dinosaur climbing apparatus on the playground. On the side closest to the building, the resilient surfacing extended approximately 1 foot 1 inch from the equipment. The required amount of resilient surfacing should be 6 feet. Children ages 2-4 were observed playing on equipment during inspection.
Plan of Correction: Children will be prohibited from using climbing apparatus and staff will speak to administration of the church/school for a resolution by 07/08/2022
