Technical Assistance:
I had a discussion with the supervisor regarding medical authorizations and action plans.

Comments:
A monitoring inspection was conducted on 7/11/2022. There were a total of 49 children with 5 staff members present. Children were observed during card playing, handwashing and lunch preparation. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-60-A
Description: Based on file review, the center did not have the number of two designated people to call in an
emergency if a parent cannot be reached, as well as the work phone number and place of employment of each parent who has custody and documentation of health information as required by 8VAC20-780-130, 8VAC20-780- 140, and 8VAC20-780-150.
Evidence:
1. Child #2 had one emergency contact list.
2. Child #3 was missing mom's employer and employer phone number as well has dad's employer and address.
3. Child #5 did not have documentation of a physical or immunizations during the inspection.

Plan of Correction: We will ask mom tonight.

Standard #: 8VAC20-780-510-E
Description: Based on a review of medication, they did not include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days.
Evidence: Child #1 had a medical authorization form that did not include effective dates.

Plan of Correction: I will have mom fill them out.

Standard #: 8VAC20-780-550-I
Description: Based on observation, 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center were not posted in a visible place at each telephone.
Evidence: Emergency numbers were not posted by the phone during the inspection.

Plan of Correction: Corrected during the inspection.
