Comments:
A monitoring inspection was initiated on 7/13/2022 and concluded on 7/13/2022. There were a total of 26 children with 6 staff members present. Children were observed during science activities and outdoor play time. A total of 3 child records and 3 staff records were reviewed. Inspector referred assistant director to the DOE website for resources and model forms. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-2
Description: Based on a review of staff records, staff did not have required fingerprint results prior to first day of employment.
Evidence:
The following staff did not have fingerprint results prior to their first day of employment. These staff were observed supervising children.
Staff #1, date of first employment 5/2/2022. Fingerprint results dated 5/4/2022
Staff #3, date of first employment 7/11/2022. There were no fingerprint results on file.

Plan of Correction: Staff will not be hired until the center receives the results of the fingerprints.

Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, each staff member has not submitted documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment.
Evidence: The following staff do not have TB results within the last 30 calendar days of their employment. These staff were observed supervising children.
Staff #1, date of employment 5/2/2022, TB completed 5/11/2022
Staff #2, date of employment 6/13/2022, TB completed 1/21/2022
Staff #3, date of employment 7/11/2022, TB completed 5/22/2022.

Plan of Correction: Staff will get TB screening 30 days prior to hire.

Standard #: 8VAC20-780-70
Description: Based on a review of staff records, documentation of two or more references were not checked prior to employment for each staff member.
Evidence: The following staff were hired with no references checked prior to employment and were observed supervising the children;

Plan of Correction: References will be obtained and filed in the staff's record.

Standard #: 8VAC20-780-510-I
Description: Based on a review of medications, a prescription medication was not maintained in the original prescription labeled container. The prescription medication did not contain the prescriber's instruction pertaining to dosage, frequency and manner of administration.
Evidence: Child #1's prescription inhaler was not in the pharmacy labeled container. The inhaler was not labeled with the Child's name nor were their directions as how to administer the medication., how much medication to administer and how often to administer.

Plan of Correction: The parent will bring in the pharmacy labeled container for the inhaler. Medications will be in the original pharmacy labeled container with the proper dosage and administration information.

Standard #: 8VAC20-780-550-D
Description: Based on a review of emergency evacuation drill and assistant director interview, the center has not implemented monthly practice evacuation drill.
Evidence: The emergency evacuation drill record did not show that a fire drill was completed for the following months; January 2022, February 2022, May 2022 and June 2022. The assistant director stated that they were not conducted.

Plan of Correction: Monthly fire drills will be conducted and documented on the form
