Technical Assistance:

+Ensure allergy lists are current and dated
+There is a blind spot between the gym and lobby restroom - ensure staff are placed where they can see children when going between these two areas
+Tables should be provided for all children when eating meals and snacks
+The latch on the playground gate is in need of repair
+The standing water at the outdoor drain exit needs to be addressed

Comments:
An unannounced monitoring inspection was conducted on 07/14/2022 from 12:00 pm - 3:05 pm. At the time of entrance there were 133 school age children in care with 12 teaching staff. Children were observed during lunch and afternoon program time. Records were reviewed for five children and five staff.
Violations were observed in the areas of staff background checks and special services. These violations are listed on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on record review and interview, the center failed to ensure that staff have a central registry finding within 30 days of employment.

Evidence:
1. Staff 1, hire date 09/08/2020, lacks the findings of a central registry check.
2. Administrative staff verified that these findings were not available for staff 1.

Plan of Correction: A central registry check has been requested for staff 1 based on an audit of staff files prior to summer camp. This request was made on 06/27/2022. The findings will be forwarded to the inspector when obtained.

Standard #: 8VAC20-780-560-J
Description: Based on interview and observation, staff failed to ensure that tables shall be sanitized prior to use for feeding.

Evidence:
1. Children were observed eating lunch at the outdoor picnic tables. The inspector asked the staff if the tables had been sanitized prior to lunch and staff stated the tables had not been sanitized.
2. Children were observed eating lunch at the tables set up in the gym. The inspector asked the staff if the tables had been sanitized prior to lunch and staff stated the tables had not been sanitized.

Plan of Correction: All staff will be re-trained on required sanitizing procedures for tables used for meals and snacks.
