Comments:
An unannounced renewal inspection was conducted on 7/1/22 from 9:00am to 12:00pm with the Administration Assistant. There were 71 children in care, ranging in age from five-months to seven-years-old, supervised by 14 staff. The children were observed eating breakfast, cleaning up their classrooms, having free play, doing puzzles and discussing shapes, playing in dramatic play, having circle time, singing and dancing, playing on the playground, and playing in centers while doing small group work activities with teachers. Six child records and five staff records were reviewed. The Center has 23 staff with current certification in CPR and First Aid, and 15 staff trained in Daily Health Observation Training. Four medications and their authorization forms were reviewed and the center has nine staff certified in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. Breakfast served was turkey sausage, egg patties, and milk. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 7/22/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-40-L
Description: Based on interview, the center did not ensure that all staff who work with children were informed of children?s allergies, sensitivities, and dietary restrictions. Evidence: In the PreK-1 (four-year-olds) class the Licensing Inspector (LI) asked the staff if any of the children had food allergies. One staff stated that they ?were new here? and did not know. The other staff stated that Child A has an allergy to pears and dairy. The record for Child A contains documentation of allergies to sweet potato and artificial grape flavor.

Plan of Correction: First day of employee training we will go over all allergies for children and where the allergy list is in each classroom.

Standard #: 8VAC20-780-40-M
Description: Based on observation, the center did not maintain a current written list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: 1. In the School Age classroom the staff reported that they did not have an allergy list. 2. In the 2?s class, the Early Preschool class (two-year-olds to three-year-olds), and the PreK-2 (four-year-olds to five-year-olds) class a written allergy list was observed that was dated November 2021. Interviews revealed that there are children enrolled who have had updates to the status of their allergies/sensitivities since November 2021 as well as new children enrolled with allergies/sensitivities. 3. In the Preschool 1 (three-year-olds to four-year-olds) class a written allergy list was observed that was dated 2019.

Plan of Correction: Going forward management will update all Allergy lists and place in the classroom. This will be updated when a new child enrolls with allergies or at least quarterly. We will ensure it is placed in correct spot in class.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of six child records, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child A contained documentation of diagnosed allergies to sweet potato and artificial grape flavor. The record did not contain an allergy care plan.

Plan of Correction: During the paper work appointment we will go over all allergies the child may have and will not enroll new children without an allergy packet.
