A monitoring inspection was initiated and concluded on July 13th. There were 71 children present ages two through nine years old with 12 staff. During the inspection a walking tour of the center inside and out was completed. A sampling of children and staff records were reviewed. Children were observed in teacher and self-directed activities including outside play, transitions, hand washing, lunch, transportation and rest period. Additionally, the fire, health inspection, emergency equipment and emergency evacuation drills were reviewed to determine if required documentation was complete. Violations regarding supervision, emergency evacuations and medication authorizations were observed, appear on the violation notice and were discussed during the exit interview at the conclusion of the inspection.

Standard #: 8VAC20-780-340-F Description: Based on observation and interview the center
failed to ensure that children under 10 years of
age always are within actual sigh and sound
supervision of staff.
Evidence:
1 - Upon entering the two year old classroom
there were four children napping in the room
alone.
2 - Staff 1 stated that she was in the restroom.
3 - The restroom is located in such a way that
sight supervision cannot be maintained of
children in the in the classroom.Plan of Correction: All staff will be reminded and retrained
regarding the need for sight and sound
supervision all times.
Standard #: 8VAC20-780-510-E Description: Based on record review the center failed to
obtain a long-term prescription drug
authorization with the child's physician and
parent signature.
Evidence:
1 - There is a long term authorization for
prescription Albuterol that did not include a
parent signature on the long term authorization
form.
2 - Staff 2 confirmed that the long term
authorization form did not include the parent
signature.Plan of Correction: The parent signature will be obtained.
Standard #: 8VAC20-780-550-D Description: Based on review of documentation and
interview the center failed to implement a
monthly practice evacuation drill.
Evidence:
1 - The written documentation of emergency
evacuation drills did not show documentation
that drills were completed during the months of
May or June 2022.
2 - Staff 2 stated that the drills were not
completed in May or June 2022.Plan of Correction: Two additional evacuation drills will be completed during the months of July and August to make up for the two drills missed.
