Comments:
A monitoring inspection was conducted on July 20, 2022. There were 21 children present, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-4
Description: Based on review of 3 staff records, one did not contain results of a criminal history record information check and a search of the child abuse and neglect registry for a state they have resided in the preceding five years.
Evidence
The record for staff # 3 (DOH 5/22/22) did not contain the results of a criminal history record information check and a search of the child abuse and neglect registry for a state they have resided in the preceding five years.

Plan of Correction: We will submit the out of state background checks immediately. When results are received we will forward them to the inspector.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of 3 staff records, 2 did not have a central registry finding within 30 days of employment.
Evidence
The record for staff #1 (DOH) 5/17/22) and #3 (DOH 5/22/22) did not have a central registry finding within 30 days of employment.

Plan of Correction: We will submit CPS checks within the first week of employment and put the results in their record. If not received within 30 days we will follow up.
The CPS for staff # 1 and #3 will be submitted immediately and results will be send to the inspector.

Standard #: 8VAC20-780-160-A
Description: Based on review of 3 staff records, 3 did not submit documentation of a negative tuberculosis (TB) screening that was completed within the last 30 calendars of employment before the first day of employment.
Evidence
Staff #1 (DOH 5/17/22), staff #2 (DOH 4/22/22) staff #3 (DOH 5/22/22) did not submit documentation of a negative TB screening before the first day of employment.

Plan of Correction: Staff #1, #2 and #3 will have their TB completed immediately. All staff will have their TB completed before their first day of employment.

Standard #: 8VAC20-780-70
Description: Based on review of 3 staff records, 2 were incomplete.
Evidence
The record for staff #1 (DOH 5/17/22) and #3 (DOH 5/22/22) did not contain documentation that 2 or more references as to a character and reputation were checked before employment.

Plan of Correction: We will complete 2 reference checks for all applicants and place the report in their record.

Standard #: 8VAC20-780-550-P
Description: Based on review of procedures for emergencies, a written record of children's serious and minor injuries was not complete.
Evidence
The written record of children's serious and minor injuries did not contain the time parents were notified and any future action to prevent recurrence of the injury

Plan of Correction: We will ensure all required information is recorded on the accident reports.

Standard #: 8VAC20-780-570-E
Description: Based on observation, bottles located in the refrigerator was not dated and labeled with the child's name.
Evidence
Bottles located in the Infant room refrigerator were not labeled with current date.

Plan of Correction: We will make sure all bottles are labeled with a child's full name and current date.
