Comments:
An unannounced monitoring inspection was conducted on July 13, 2022. There were approximately 70 children in care with 10 staff. Children present ranged in age from two to nine years old. Children and staff records were reviewed. A walking tour of the facility inside and outside was completed. Children were observed in teacher and self-directed activities including but not limited to outside play, lunch, transitions and restroom routines.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation the center failed to ensure that areas are maintained in safe condition.
Evidence:
In one out of seven classrooms observed there is a dangling electrical cord within reach of children. The cord is from a wall mounted TV and poses a toppling hazard.

Plan of Correction: The dangling cord will be covered in such a way that it is no longer within reach of children.

Standard #: 8VAC20-780-510-P
Description: Based on review of medication the center failed to ensure that medications are returned to parents or disposed of properly when medications are not picked up within 14 days.
Evidence:
There were Epi Pen and Benadryl in the medication locked location for child 1 who left the center's care on June 14, 2022.
There was an Epi Pen in the medication locked location for child 2, the authorization expired June, 2022.

Plan of Correction: All medications will be returned to parents or properly disposed of.

Standard #: 8VAC20-780-560-J
Description: Based on observation and interview the center failed to ensure that tables are sanitized before and after each use.
Evidence:
In two classrooms staff were observed spraying the tables with a solution and immediately wiping the tables dry. When asked staff reported that the solution was bleach and water. However, the tables were not allowed to air dry as required by definition, for sanitation to be complete.

Plan of Correction: Center management will remind and retrain all staff in the need for tables to be properly sanitized before and after meals snacks including allowing for 2 minutes of air drying before the sanitizer is wiped dry.
