Comments:
Today an unannounced Monitoring Inspection took place between the hours of 10:00am and 12:45pm. There were 2 classrooms observed with a total of 20 children with 5 staff within the supervision guidelines. The children were observed leaving the building to play on the playground, playing with playdough, eating lunch and other organized activities. Hand washing procedures were observed. A complete inspection of the physical plant, children and staff records, and emergency procedures were observed during this inspection. None of the staff are currently trained to administer medication and the program director stated that there is no medication kept on site. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review, the child day program did not obtain a central registry finding within 30 days of employment for each employee.
Evidence: Staff #6 (date of hire: 6/10/2022) file does not contain central registry background check results. The program director confirmed that they did not follow-up regarding with the office of background investigations when the background check results were not received within 30 days.

Plan of Correction: I did email OBI last week. I can follow-up again about the results.

Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not documentation that each child has received the immunizations before the child can attend the center.
Evidence: Child #5's record does not contain documentation of immunizations.

Plan of Correction: I'll give the parents two weeks to provide the immunizations.

Standard #: 8VAC20-780-130-E
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child #2's (23 months) most recent documented immunizations are from when the child was 15 months old.
2. Child #3's (16 months) most recent documented immunizations are from when the child was 8 months old.

Plan of Correction: I'll give the parents two weeks to provide the immunizations.

Standard #: 8VAC20-780-60-A
Description: Based on record review, children's record do not contain all required information.
Evidence: Child #6's (date of enrollment: 2/7/22) file does not contain documentation that the child's age and identity was verified within the first 7 days of enrollment.

Plan of Correction: I'll request the proof of ID from the family.

Standard #: 8VAC20-780-70
Description: Based on record review, staff records do not contain all required information.
Evidence: Staff #6 file does not contain documentation of two reference checks.

Plan of Correction: I can get that done this week.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children.
Evidence: Two air fresheners were observed in the bottom cabinets of the diaper changing table. The cabinet door was not locked.

Plan of Correction: We can move them.

Standard #: 8VAC20-780-500-B
Description: Based on observation, diapering requirements were not met.
Evidence: The diaper pail used for used diapers is not foot operated.

Plan of Correction: We have a foot operated garbage in the classroom. I'll swap the cans so that the foot operated is next to the changing table.

Standard #: 8VAC20-780-560-G
Description: Based on observation, food brought from home was not clearly labeled to identify it's owner and/or dated.
Evidence:
1. Child #7 had a food container stored in the infant classroom refrigerator that was not dated.
2. Child #8 had a food container stored in the infant classroom refrigerator that was not dated.
3. Two lunch boxes were observed to be handed to the incorrect children.

Plan of Correction: I have reminded the parents about the policy and the staff know to be looking at bottles and foot. They have a sharpie marker in the classroom to be labeling. I'll reinforce the policy again.

Standard #: 8VAC20-780-560-M
Description: Based on observation, children were allowed to eat while walking around.
Evidence: At the start of the inspection, a group of children was observed leaving the classroom to walk to the playground. Child #1 was observed being handed a lollipop to join the group and was observed walked down the hall with the lollipop in their mouth.

Plan of Correction: I'll speak to the child's mom.

Standard #: 8VAC20-780-570-A
Description: Based on observation, children placed in an infant seat or high chair, the protective belt was not fastened securely.
Evidence:
1. Child #1 was observed sitting in a high chair and climbed out of the chair twice during lunch.
2. Child #2 was observed sitting in a high chair without being buckled during lunch.

Plan of Correction: I'll go over equipment policies with the staff again.

Standard #: 8VAC20-780-570-E
Description: Based on observation, prepared infant bottles were not dated.
Evidence: Child #7 had two bottles stored in the infant room refrigerator. The bottles were not dated.

Plan of Correction: I have reminded the parents about the policy and the staff know to be looking at bottles and foot. They have a sharpie marker in the classroom to be labeling. I'll reinforce the policy again.
