Comments:
An unannounced monitoring inspection was conducted on July 19, 2022 from approximately 9:44 am to 11:05 am. There were 16 children in attendance and a total of 4 staff present. 3 staff records and 4 children?s records were reviewed. All areas of the center were observed including classrooms, hallways, kitchen, and bathrooms. The children were observed playing with toys and puzzles, reading, playing outside, and washing hands while the licensing inspector was on the premises.

Emergency drills and inspections were in compliance. The most recent Health Inspection was on 10/05/21. The most recent Fire Inspection was on 04/10/2022; Fire Drills are conducted monthly with the most recent on 06/13/22. Shelter in Place Drills were conducted twice with the most recent on 06/13/2022. The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted by each phone.

Violations were cited as a result of this inspection.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not maintain areas of the center outside in a safe condition. Evidence: 1) There was an unsecured 2inch by 4 inch by 6 foot plank of wood leaning up against the mulched area of the playground. 2) The music structure had rough un-sanded wood with sharp screws sticking out of it. 3) There was an open box with pieces to an unassembled play structure that contained loose screws and wooden pieces with sharp edges.

Plan of Correction: Per the center "due to the original builder being injured and unable to complete the playground, a new person was contracted to complete/repair all projects."

Standard #: 8VAC20-780-340-C
Description: Based on observation, interview, and staff records review, the center did not ensure that there will always be on the premises when children are present one staff member who meets the qualifications of program leader or program director. Evidence: The inspector arrived at 9:44 am to conduct the inspection. The Program Director had just left to go to an appointment leaving the Assistant Director in charge. At approximately 10:30 am the Assistant Director left to take a group of children on a field trip, leaving two school-age teachers alone with a group of children. When asked who was in charge staff #1 stated that staff #2 was in charge. Staff #2?s staff record did not include documentation to demonstrate the qualifications for program leader. When interviewed the Director stated that she was aware that staff #2 did not meet the qualifications.

Plan of Correction: Per the center, "I have advised my staff that in any similar situation in the future, the Preschool group would simply have to reschedule their field trip for another day, to accommodate for the employees with the necessary qualifications that are missing."

Standard #: 8VAC20-780-550-P
Description: Based on review of documentation, the center did not maintain a written record of the day and time when parents are notified of children?s injuries. Evidence: 12 out of 12 injury reports viewed had written ?at pickup? rather than the actual time the parent was notified.

Plan of Correction: Per the center, "staff have been retrained to document the actual time."
