Technical Assistance:
Clarified the requirements for CPR and First Aid orientation and certification.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program. A monitoring inspection was initiated on July 26, 2022 and concluded on July 28, 2022. There were 33 children present, ranging in ages from 10 to 14, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and staff interviews, the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening and the documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: One out of five records reviewed did not have documentation of TB screening/ test conducted in the required timeframe. Staff #2?s TB screening was dated October 26, 2021 and the first date of employment was February 13, 2022.

Plan of Correction: In future TB test will be done ahead of time 7-28-22

Standard #: 8VAC20-780-160-C
Description: Based on record review and staff interviews, the licensee did not ensure that each staff member had obtained at least every two years from the date of the initial screening or testing, the results of a follow-up tuberculosis screening.

Evidence: The record for Staff #5?s did not include an updated TB screening as the most recent one was dated February 26-2019.

Plan of Correction: Will have TB results by Saturday by 7-30-202

Standard #: 8VAC20-780-240-A
Description: Based on review of documentation and staff interviews, the licensee did not ensure employees had obtained the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence: Staff #2?s date of hire February 13, 2022 does not have documentation of the required training. Center staff confirmed staff #2 had not completed the training.

Plan of Correction: Staff will get the training by August 19, 2022

Standard #: 8VAC20-780-510-G
Description: Based on review of documentation and staff interviews, the licensee did not ensure that medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: An albuterol inhaler was observed and it did not have the child's name, the dosage amount, and the time or times to be given. Center staff confirmed the albuterol inhaler belonged to child #3 and it had not been administered.

Plan of Correction: Medication will be returned to parents.

Standard #: 8VAC20-780-510-P
Description: Based on review of documentation and staff interviews, the licensee did not ensure when an authorization for medication expires, the parent shall be notified the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Evidence: An epi-pen for child #4 was observed. The authorization for the medication was labeled for ?school year? and it was signed by the physician on April 26, 2022. The medication had not been sent home to the parents or disposed of as required.

Plan of Correction: Medication will be returned to parents. A new medication authorization will be required.
