Comments:
An unannounced monitoring inspection was conducted today, August 2, 2022. There were 11 children present with 3 staff supervising. The physical plant, emergency preparedness procedures, medications and authorizations, allergy lists, playground area, staff records and children's records were reviewed. The children were observed completing morning snack, hand washing, working on art projects and going to the playground. Area's of non-compliance were cited on the Violation Notice. Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a review of children's records, the center failed to ensure that all required documentation was obtained for each child enrolled.

Evidence:
1. Child #1, #2 and #3's records did not contain documentation of one parent's work phone number.
2. Child #1 and 2's records had documentation of authorized pick up persons that are all out of state at a distance that would make pick up not possible in an emergency.
3. Child #1's record did not contain documentation that an annual review of emergency contact information has been completed within the last year.

Plan of Correction: I will get this information from the parents.

Standard #: 8VAC20-780-540-C
Description: Based on observation, the first aid kit did not contain all required supplies.

Evidence:
The first aid kit was missing a second triangular bandage and a First Aid manual.

Plan of Correction: I will purchase an additional triangular bandage and a first aid manual for the first aid kit.

Standard #: 8VAC20-780-550-C
Description: Based on review, the procedures for both emergency evacuation and shelter-in-place were not posted in a location conspicuous to staff and child on each floor of the building.

Evidence:
On the date of the inspection, the Emergency Preparedness Plan (EPP) was not posted on either floor of the center. The EPP was observed in a binder kept in the office.

Plan of Correction: I will post the Emergency Preparedness Plan on both floors of the building.
