Comments:
An unannounced renewal inspection was conducted on 8/2/2022. There were 23 children, ages 9 months -4 years and 5 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 3 staff records were reviewed. Board Member backgrounds reviewed. The children were observed during active time on the playground, center time and circle time. The inspector arrived for the inspection at 9:45am and departed the center at 11:53 am.

Discussion on standards related to staff records.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on review of three children?s records, the center failed to ensure that all physical examination records were obtained before the child?s attendance or within 30 days after attendance.

Evidence: Child #1 first date of attendance was 05/05/2022 and on the day of the inspection did not have the required physical examination record.

Plan of Correction: Provider will obtain physical examination record from parent.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that equipment on the playground was clean, safe and operable.

Evidence: Observed 2 pieces of broken equipment on the playground.
1. Three seats on the blue and red carousel play structure were cracked from the rear to the middle of the seat causing a pinch point. Three children in the toddler classroom were observed playing on the structure.
2. The playhouse structure with the green roof was cracked on the left wall under the plastic play sink causing a pinch point. There were no children observed in the playhouse but it was accessible to the children.

Plan of Correction: Provider will replace the three seats on the blue and red carousel structure and will remove the playhouse with the green roof from the playground.
