Comments:
An unannounced complaint inspection was conducted on 07/20/2022. The inspector interviewed 7 staff members, and reviewed 3 children?s records and 5 staff records during the investigation. The information gathered during the inspection did not support the complaint and therefore the complaint was determined to be not valid. However, information gathered during the course of the investigation did determined non-compliance with applicable standards or law and violations unrelated to the complaint were documented on the violation notice issued to the provider.

Description: Based on record review and discussion, the center failed to ensure that each staff record contains all of the elements as required by the standards.

Evidence:
1. The record for Staff 1 did not contain the following information: address, job title, date of hire, and name, address and telephone number of a person to be notified in an emergency; documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the education, certification and experience required by the job position and the required orientation training.
2. The record for Staff 2 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the required orientation training.
3. The record for Staff 3 did not contain the following information: job title, date of hire; documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the education, certification and experience required by the job position and the required orientation training.
4. The record for Staff 4 did not contain documentation to demonstrate that the individual possesses the required orientation training.
5. The record for Staff 4 did not contain the following information: job title, date of hire; documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the education, certification and experience required by the job position and the required orientation training.

Description: Based on observation and discussion, the center failed to ensure that the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: In the large ?playroom? used for the infant classroom there was a piece of the sheetrock missing from the wall near the outside exit door that exposed the insulation inside of the wall; the insulation was accessible to the children in the room. Children were observed using this space during the inspection.

Plan of Correction: The wall has been repaired.
