Comments:
An unannounced complaint inspection was initiated on 06/10/2022 and completed on 07/29/2022 in response to an allegation of a forbidden action. Based on the information gathered which included interviews and a review of the facility the complaint regarding the forbidden action was found to be not valid.
Violations were found in the area of behavior guidance and supervision and are listed on the violation notice issued to the facility.

Description: Based on staff interviews and a review of the facility, the center failed to ensure that children under 10 years of age shall be within actual sight and sound supervision of staff.

Evidence:
1. Child 1, age four years, was left alone, crying in the first hallway bathroom, to change his clothing after a toileting accident at approximately 4:30 pm on 06/08/2022.
a. Staff 1, who was responsible for child 1, stated she left child 1 alone in the bathroom to change his clothing.
2. Staff 1 was in a classroom with 3 children during this time watching a video. The location and distance of this classroom from the first hallway bathroom does not allow for sight or sound supervision.
a. This bathroom is an enclosed single bathroom located around the corner and approximately 21 feet from the classroom where staff 1 was.
3. Staff 1 stated she did not provide intermittent sight supervision or assist the child in the bathroom to clean himself up.

Plan of Correction: Staff will be retrained on actual sight and sound supervision. Training to take place no later than 8/19/2022.

Description: Based on staff interviews, the facility failed to promote a child's emotional and social well-being and growth and staff shall interact with the child and one another to provide needed help, comfort and support.

Evidence:
The facility failed to provide for the needed help and comfort of a four year old child based on the following incident which occurred on 06/08/2022 at approximately 4:30 pm:
1. Child 1 was left alone in a bathroom to change his clothing after a toileting accident which occurred on the facility outdoor play area.
a. Staff 1 and staff 2 stated that child 1 was crying and upset.
b. Child 1 stated he was crying, "I was wet inside and I peed on myself and I was crying."
2. Staff 1 confirmed that she provided child 1 with a change of clothing and a pull-up and told him to change his clothing.
a. Staff 1 stated she left child 1 alone in the bathroom to change his clothing and did not assist him until he came into the classroom with the pants buttoned incorrectly.
b. Staff 1 further indicated she raised her voice and asked child 1, "Why do you keep peeing on yourself?"
3. Staff 1 and staff 2 stated that child 1 frequently has toileting accidents and was further upset when they stated his parent would be informed of the accident.

Plan of Correction: Staff will be retrained on Behavior Guidance. Training to take place no later than 08/19/2022.
