A monitoring inspection was conducted on 8/08/2022 with the center director and center staff. There were 65 children present, ranging in ages from 7 months to 12 years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 5 staff records were reviewed. The children were finishing lunch, preparing for rest or quiet activities, and swimming in the pool.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-770-60-B Description: Based on review of five staff records, the facility failed to have documentation of a completed sworn disclosure statement on file by the first day of employment. Evidence: Staff 1 (date of hire 7/25/2022) did not complete a sworn disclosure statement until 7/30/2022. Plan of Correction: Each new staff will complete the sworn disclosure statement on the first day of employment.
Standard #: 8VAC20-780-160-A Description: Based on review of five staff records, the facility failed to have a tuberculosis test or screening for each staff prior to employment or direct contact with children. Evidence: Staff 1 (date of employment 7/25/2022) did not get a tuberculosis test until 8/03/2022. Plan of Correction: Each new staff will provide documentation of a tuberculosis test or screening before direct contact with children.
Standard #: 8VAC20-780-280-G Description: Based on observation, the facility failed to label all hazardous substances in substitute containers with the contents. Evidence: there was a spray bottle in the school age girls bathroom that was not labeled with the contents. The center director said it was probably air freshener. Plan of Correction: All substitute containers will be labeled with the contents.
Standard #: 8VAC20-780-320-B Description: Based on observation, the facility failed to equip each restroom with all required supplies. Evidence: the school age girls restroom did not have toilet paper during today's inspection. Plan of Correction: Toilet paper was put in the girls restroom. Staff will consistently check to make sure the bathroom is stocked with necessary supplies.
