Comments:
A monitoring inspection was conducted on 8/2/2022 with the center director and center staff. There were 72 children present, ranging in ages from 2 months to 12 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 7 child records and 7 staff records were reviewed. The children were having rest time, afternoon snack, and free play time.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on a review of seven staff records and staff interview, the center did not obtain a repeat search of the central registry (CPS check) for one staff member within five years, as required.
Evidence: The record for Staff 6, (date of employment 3/6/2017) contains documentation of a central registry finding dated 4/07/2017. The central registry search should have been updated within 5 years (by 4/07/2022). The center director confirmed the repeat central registry check had not been submitted as of 8/2/2022.

Plan of Correction: The repeat CPS check will be submitted as soon as possible.

Standard #: 8VAC20-780-270-A
Description: Based on observation and staff interview, the facility failed to maintain all areas in a safe and operable condition. Evidence: On 8/2/2022, the grass on the areas in and surrounding the playground was 8 to 9 inches tall. This made it difficult for the children to easily run and play and could harbor ticks. The center director stated the grass is typically cut every other week.

Plan of Correction: Center staff will see if the lawn can be cut more frequently.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the facility failed to utilize a leakproof, foot operated diaper disposal system (or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal). Evidence: the pull up diapers in the twos bathroom were observed in an open trash can, without a lid.

Plan of Correction: All diapers will be disposed of in a lidded trashcan that is foot operated.

Standard #: 8VAC20-780-530-A
Description: Based on a observation and review of staff files, the center failed to ensure that there was one staff member present in each grouping of children who is certified in cardiopulmonary resuscitation (CPR) and first aid. Evidence: In the Montessori classroom, Staff 1 was observed alone in a grouping of children. There was no documentation to support that Staff 1 is certified in CPR or first aid. The center director confirmed Staff 1 did not have certification in CPR or first aid.

Plan of Correction: Each grouping of children will have a staff with current certification in first aid and CPR.
