Comments:
A monitoring inspection was conducted on August 23, 2022 There were 66 children present, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and seven staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-770-60-C-2
Description: Based on review of seven staff records, one does not have a central registry finding within 30 days of employment.
Evidence
Staff #7 (date of hire 6/28/22) does not have a central registry finding in their record. Follow up was not completed after 30 days, follow up on missing central registry finding was conducted on 8/18/22.

Plan of Correction: We will continue to follow up on the where the results are. We will let our inspector know when the results are received.

Standard #: 8VAC20-780-160-A
Description: Based on review of seven staff records, one contained documentation of a negative tuberculosis (TB) screening that was not completed within the last 30 calendar days of the date of employment.
Evidence
The record for staff # 1 (date of hire 3/1/22) contained a TB screening that was not dated within 30 days of first day of employment. TB screening dated 1/14/21,

Plan of Correction: We will make sure all TB screenings are within 30 days of first day employment.

Standard #: 8VAC20-780-550-P
Description: Based on review of the centers written record of children's serious and minor injuries, the written record does not include all required information.
Evidence
The centers written record of children's serious and minor injuries does not include any future action to prevent recurrence of the injury.

Plan of Correction: We will add how to recent future occurrence to our form and retrain all staff.
