Technical Assistance:
Technical assistance was provided in the following areas of the standards: Background checks, child records, staff records, Program Leader qualifications, supervision, first aid/emergency supplies, emergency drills and procedures, and infant feeding.

Comments:
An unannounced monitoring inspection was conducted on 8/23/22 from 9:30am - 11:15am. During the inspection there were 66 children ages three months old through twelve years old in care with ten staff. Children were observed participating in various activities in the classrooms. Records were reviewed for six children and six staff. Medication, emergency procedures, and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, it was determined that the facility did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. The cabinet over the sink in the Two's classroom was not locked and contained several bottles of hazardous substances used for cleaning.
2. Staff #7 confirmed there were hazardous substances present in the cabinet that were not kept in a locked place.

Plan of Correction: The facility responded: Corrected during the inspection. Staff will be reminded to store all hazardous substances in a locked location.

Standard #: 8VAC20-780-350-B-5
Description: Based on on observation and interviews, it was determined that the facility did not ensure that for children, from the age of eligibility to attend public school through eight years, one staff member for every 18 children is maintained at all times children are in care.

Evidence:
1. There were 20 children in the School age classroom with one staff. The staff present in the classroom stated that the youngest child present was five years old.
2. Staff #7 confirmed that the facility did not maintain the required staff-to-child ratio of one staff for every 18 children..

Plan of Correction: The facility responded: Corrected during the inspection. Staff will be reminded to maintain the correct staff-to-child ratios required by the Licensing regulations.

Standard #: 8VAC20-780-560-G
Description: Based on interviews, it was determined that the licensee did not ensure that when food is brought from home it is labeled with the child's name and date.

Evidence:
1. There were bottles of formula in the refrigerator located in the Infant classroom that were not labeled with the child's name or date.
2. Staff #7 confirmed that the bottles for the children in care were not labeled with the child's name or date.

Plan of Correction: The facility responded: Corrected during the inspection. Staff will be reminded to labeled all bottles with the child's name and the date.
