An unannounced complaint inspection was conducted on site on 7/5/2022,7/12/2022 and 7/28/2022. The inspectors arrived at 12:00 PM and departed at 1:45 P.M. Follow up interviews were completed on 7/12/2022 and 7/28/2022. A complaint was received in the licensing office on
6/28/2022. The preponderance of evidence gathered did support forbidden action and complaint is valid.

Standard #: 8VAC20-780-350-B-5 Complaint related: Yes Description: Based on interviews and attendance records, the center failed to ensure that the staff to child ratio was met.
Evidence:
1.Staff # 1 and Staff #2 were on the playground with 25 children ages 5-12 years old. The electronic attendance record confirms the number of children in care.
2.Staff # 2 left Staff 1 on the playground with 25 children to go inside the center. Staff #3 stated Staff #2 was in the building for approximately 10-12 minutes.
3.Staff # 1 stated that they were frustrated about being left out of ratio.Plan of Correction: Staff # 1 was let go. Staff # 2 was written up. Issue addressed with all staff to not be out of ratio.
Standard #: 8VAC20-780-410-1 Complaint related: Yes Description: Based on video evidence, the center failed to ensure that staff did not participate any forbidden action.
Evidence
1.On June 29, 2022 the video showed the school age class on the playground with Staff # 1. Staff # 1 pulled child # 1 by the arm while walking approximately 15 feet and then the Staff# 1 pulled the child?s arm down until the child was sitting on the ground.Plan of Correction: Staff # 1 was let go. All staff received additional training on playground safety.
