Comments:
An unannounced complaint investigation was conducted on 08/12/2022 regarding administration, physical plant, programs and special care provision and emergency . Interviews and observations were conducted and records were reviewed. There was preponderance of evidence support the allegations in regards to physical plant and programs and the complaint has been deemed valid.

Description: Based on observation and staff interview, the licensee did not ensure the center was clean , safe and operable.

Evidence:
On August 12, 2022, staff confirmed they did not have hot water. On August 17, 2022 , the owner confirmed the hot water heater was turned off and there was no hot water.

The bathrooms on the right side of the building, one bathroom had gloves on the floor a potty training seat on the floor and trash. The second bathroom, had a diaper hanging out of the trash can.

In the school age, pre-k and infant hallway, there was a bathroom with a trash can overflowing of used parper towels.

In the kitchen, there was two boxes of trash and trash bags over flowing of trash.

In between the toddlers and two?s classroom, there is an AC unit cord that is hanging and is a tripping hazard.

In the three year old classroom, there is an AC unit cord hanging that is a tripping hazard.

In between the toddlers and two?s classroom, there was a vent cover that was broken that is sharp and can entangle shoe laces.

Plan of Correction: The hot water was fixed on August 19, 2022.

There were cleaning procedures and charts that staff will need to sign off through out the day for the bathrooms.

In the kitchen, there is a cleaning checklist and there should never be trash outside of the trash can at anytime and trash needs to go outside.

The AC unit cords were duct taped to the walls.

The vent is covered over with a shelf.

Complaint related: Yes
Description: Based on observation licensee did not ensure there the mats and cots were identifired for specific children.

Evidence:
In the three year old classroom, the staff confirmed the mats/cots were not labeled.

Plan of Correction: Mats will be replaced by cots. There will be a label on each cot with the child's name. There will be a chart posted in classroom where cots will are placed.
