The following was discussed with the program director:
400-B Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
There should be enough infant emergency supplies to include bottles and nipples for each enrolled child.

A monitoring inspection was conducted on 08/26/2022. There were 78 children present and 14 staff. A tour of the facility was conducted, interviews with staff and a record review. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Standard #: 8VAC20-780-270-A Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
There was peeling paint on the back wall of the bathroom stalls in the preschool room.Plan of Correction: The director will put in a work order to have maintenance correct this.
Standard #: 8VAC20-780-340-A Description: Based on observation and interview the center did not ensure when staff are supervising children, they are always ensuring their care, protection and guidance.
Evidence:
When the inspector walked into the 2 year-old classroom there were 14 children present. Staff A had her back turned while changing a child's diaper and Staff B did not look up from across the room to notice the inspector walked in.
Upon entering, 2 children were observed at the sink drinking water that had backed-up and had a piece of toilet paper floating in it. A third child was standing at the door with her pants around her ankles and asked the inspector for help in the bathroom.
When the inspector brought to the staff's attention, children were in need of assistance, Staff A turned her back to the child on the changing table and took her hand off of him.Plan of Correction: The director stated the center was short staffed. The lead in this class was on vacation and the assistant requested time-off. There were 2 staff that were not the regular caregivers working in that room. The regular staff will be returning Monday. In the meantime, the director appointed a stronger lead switch classes to assist in this room.
Standard #: 8VAC20-780-430-I Description: Based on observation the center did not ensure personal articles were individually assigned.
Evidence:
A school-age child was observed taking a tube of lip balm from a cubby and applying it to her lips. The tube was not labeled in anyway that identified the owner.Plan of Correction: The director will speak with staff about how to handle over-the-counter skin products.
Standard #: 8VAC20-780-500-A Description: Based on observation the center did not ensure all handwashing requirements were met.
Evidence:
In the 2 year-old room Staff A was observed changing a child's diaper. She put the child down, gathered soiled items from the table, using the handle, opened the bathroom door and discard the soiled items. Staff A did not wash her hands after she changed the diaper.Plan of Correction: The staff in the room was not the regular caregiver, however all staff in the building will be retrained on proper diapering and handwashing.
Standard #: 8VAC20-780-500-B Description: Based on observation the center did not ensure all requirements were met when diapering.
Evidence:
In the 2 year-old room, Staff A did not discard of soiled diaper and linens in either foot-operated trash can or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.Plan of Correction: The staff in the room was not the regular caregiver, however all staff in the building will be retrained on proper diapering and handwashing.
