An unannounced Monitoring Inspection took place on 08/24/22 between the hours of approximately 10:30 a.m. and 12:00 p.m. There were 4 classrooms observed with a total of 14 children with 4 staff within the supervision guidelines. The children were observed participating in free play and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-60-A-8 Description: Based on record review, the center did not obtain all information for children?s records.
Evidence: Child #2?s record indicates an allergy to dairy. The record for Child #2 did not include a written allergy care plan.Plan of Correction: I will ask the parent to submit an allergy care plan for the child.
Standard #: 8VAC20-780-80-A Description: Based on record review, for each group of children, the center did not maintain a written record of attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: When attendance records were reviewed at approximately 11:05 a.m., Child #4 had not been checked in. Staff # 4 stated that Child #4 arrived at approximately 7:20 a.m.Plan of Correction: Corrected during inspection.
Standard #: 8VAC20-780-280-G Description: Based on observation, the substitute container for a hazardous substance did not clearly indicate its contents.
Evidence: A spray bottle in the two year old classroom containing a cleaning solution did not clearly indicate its contents.Plan of Correction: We will label the bottle.
Standard #: 8VAC20-780-520-A Description: Based on observation, over-the-counter skin products were kept beyond the expiration date of the product.
Evidence:
1. Diaper cream belonging to Child #5 expired 12/2021.
2. Diaper cream belonging to Child #6 expired 02/2022.Plan of Correction: We will send the diaper creams home today and ask for new ones.
Standard #: 8VAC20-780-550-D Description: Based record review and staff interview, the center did not implement a monthly practice evacuation drill.
Evidence: The center did not implement a practice evacuation drill in April 2022.Plan of Correction: We will do fire drills every month.
