A monitoring inspection was initiated in response to a self-reported concern regarding the center?s policies and parental involvement that occurred at the center on April 26, 2022. The department was notified of the incident on April 27, 2022. The investigation was initiated on April 27, 2022 and completed on August 15, 2022. On April 28, 2022, two licensing inspectors conducted an unannounced inspection from approximately 4:00pm until 4:50pm. Interviews were completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted off-site and additional information was submitted to the inspector electronically. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 8VAC20-780-420-E-4 Description: Based on interviews and documentation reviewed, the center did not ensure that parents were informed of reasons for termination of services for two out of three children enrolled.
Evidence: 1) On 04/26/2022, the parents of Child #1, Child #2, and Child #3 were notified of termination of services from the center effective 05/04/2022. A statement signed by management and dated 04/26/2022 stated: ?Pursuant to our parent handbook, we are having to terminate services due to hazardous circumstances surrounding the behavior displayed by [Child #1].? The statement did not indicate the reasons for termination of services for Child #2 and Child #3. A statement signed by management and dated 04/29/2022 stated: ?After careful consideration, we will be terminating care effective May 4th 2022 for all three children, [Child #2], [Child #3], and [Child #1] as listed in our original letter.?
2) During an interview, Member of Management #1 stated there was no reason for termination of services for Child #2 and Child #3.Plan of Correction: We will put an addendum in our parent handbook giving more specific information on reasons for termination from our program. Termination letters will be more specific in the future. The owner and director will be responsible for making sure termination letters are properly communicated.
