An unannounced monitoring inspection was conducted on 08/29/2022. There were 30 children, ages 2 years-4 years, and 8 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during learning time, free choice activities, arts and crafts, hand washing and lunch. A total of 5 children?s records and 5 staff records were reviewed. The inspector arrived to the center at 9:30 am and departed at 11:30 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-770-60-B Description: Based on record review, the center failed to ensure that an employee must not be employed until the agency has the person?s completed sworn statement or affirmation.
Evidence: The record for Staff 3 contained a documented hire date of 05/01/2022. The sworn statement in the record for Staff 3 was not complete.Plan of Correction: Will get it completed today.
Standard #: 8VAC20-780-60-A Description: Based on record review, the center failed to ensure that the separate record for each child enrolled contains all of the elements as required by the standards.
Evidence: The record for Child 1 contained documentation of a diagnosed food allergy. There was not a written care plan from a physician regarding the food to which the child allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.Plan of Correction: Will request the information from the parent/guardian.
