Comments:
An unannounced monitoring inspection was conducted jointly with Prince William County Child Protective Services beginning 8/5/2022 and ending 8/30/2022 to investigate a self-reported incident regarding behavioral guidance and forbidden actions. Staff and child records were reviewed, staff and child interviews were conducted and the center's video recording was reviewed. No violations were found relating to the initial self reported incident, however, a violation regarding a separate incident was found during the course of the investigation regarding rough handling.

Standard #: 8VAC20-780-410-1
Description: Based on interviews and review of the center's video recording, it was determined a staff member engaged in the forbidden action of rough handling.

Evidence: On 8/5/2022 at approximately 11:20am, Staff #2 observed Child #1 (Age 7) throwing clay across the room. Staff #2 called to Child #1 across the room and asked what was in their hand. At approximately 11:21am on 8/5/2022 Child #1 was observed via video recording walking towards Staff #2. As Child #1 gets close Staff #2 roughly grabs Child #1's left hand pulling on their arm so the child has to step back. Staff #2 attempted to pry the item from Child #1's hand until the child let go of the item. Child #1 was then observed via video recording holding their left wrist while crying and then going to sit down at a table with their head down.

Plan of Correction: Staff #2 will have training on "Keeping Your Composure During Challenging Situations." Staff #2 viewed footage to understand "Perspective" and Best Practices in handling challenging behavior. School Staff retrained on "Guiding Difficult Behaviors" and directors supported teachers with information about perspectives and awareness of allowing children to independently retrieve from the emotional state of being that they are in.
