Comments:
A monitoring inspection was initiated on 8-29-22 and concluded on 8-29-22. There were 28 children present with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. The inspection started at 8:45am and concluded at 12:45pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of children files the facility failed to maintain compliance with this standard as required relating to emergency contact addresses.

Evidence:

Child file #5 did not have the addresses of either of the 2 required emergency contacts.

Plan of Correction: This information will be obtained and added to the file.

Standard #: 8VAC20-780-70
Description: Based on review of staff files the facility failed to maintain compliance with this standard as required related to employee references.

Evidence:

Staff file #1-4 did not contain 2 proof of reference verifications as required. There were no references in either of the 4 staff files. All 4 staff were working at the time of inspection.

Plan of Correction: The completed staff references will be located and placed in the file.

Standard #: 8VAC20-780-80-A
Description: Based on review of the classroom documents the facility failed to maintain compliance with this standard as required.

Evidence:
There was no written record of daily attendance, documenting arrival and departure, of the children actively in care at the time of the inspection in the following rooms as required:
1. 3-4 year class
2. 4-5 year class
3. 6-24 month class

Plan of Correction: Staff will be reminded to fill out the attendance forms upon arrival of the children and departure of the children. These forms were completed during the inspection as well.

Standard #: 8VAC20-780-280-B
Description: Based on review of the physical plant the facility failed to maintain compliance with this standard as required.

Evidence:
In the 3-4yr class and the 4-5yr class the cleaning storage closets had the key in the lock, accessible to the children, and both doors were also unlocked. Both closets contained hazardous cleaning materials.

Plan of Correction: Staff will be reminded to ensure the cleaning storage closets are locked at all times and that the key is not left within the reach of the children.

Standard #: 8VAC20-780-330-B
Description: Based on review of the playground the facility failed to maintain compliance with this standard as required related to fall zones, and resilient surfacing.

Evidence:
1. On the playground there were swings with no resilient surfacing under or around the structure, there was no 6' fall zone around the structure, and there was not the approved footage in front of or behind the swings as required by the ASTM guidelines.
2. The resilient surfacing under the big climbing structure did not measure 6" as required; it measured approximately 3".

Plan of Correction: The swings will not be used by the children until they meet compliance completely.

Standard #: 8VAC20-780-510-I
Description: Based on review of medications the facility failed to maintain compliance with this standard as required related to administration of medication according to manufacture instructions.

Evidence:
1. Children A-E had infant Tylenol or equivalent on site in the locked medication box at the time of inspection.
2. All children were under the age of 2.
3. The manufacture dosage instructions state "children under 2 consult physician".
4. All the children had only parental permission forms for administering and no doctor permission for dosage as required by manufacture instructions.

Plan of Correction: These medications will be sent home or disposed of and in the future manufacture instructions will be followed and a physician order will be obtained for these types of medications. The medication is not actively being administered.

Standard #: 8VAC20-780-510-P
Description: Based on review of medication present at the time of inspection the facility failed to maintain compliance with this standard as required.

Evidence:
1. Medication found in the locked medication box for children A-G had expired parental permission to administer forms. The medications had not been disposed of or returned to the parent and the allowed 14 days to do so had past. The medication forms all expired between March and August 2022.

Plan of Correction: These medications will be disposed of or sent home with the parents.
