Comments:
An unannounced renewal inspection was conducted today, August 31, 2022 to determine the centers compliance with licensing standards. Most but not all standards were reviewed during today's inspection. On August 31, 2022, the licensing inspector was on site from approximately 2:45 pm to 4:10 pm. The Director assisted with the inspection. There were 28 children present, ranging in ages from five years to 11 years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing a group game, having snack and crafting bookmarks. Staff interacted with children in a positive manner. A total of three child records and two staff records were reviewed. The inspector interacted with staff and children in care.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have questions regarding this inspection, please contact the licensing inspector.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of three children?s records, the center did not obtain all required information for one child?s record.

Evidence:

1. The record of child #2, enrolled 08/29/2022, did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
2. The record of child #2 had the parents listed as the emergency contacts.

Plan of Correction: Per the administrator; We will follow up with these families to ensure that we have the correct information, and they update their information in Playerspace. We will also continue to train our full-time directors on reviewing and ensuring all forms in Playerspace are filled out correctly.

Standard #: 8VAC20-780-245-J-3
Description: Based on review of medication and staff records, the center did not ensure any child for whom emergency medications (such as albuterol and epinephrine auto injector) have been prescribe was always in the care of a staff member or independent contractor who meets the requirements to perform medication administration.

Evidence:

1. Child #3, who has two prescribed emergency medication was in care on 08/31/2022 without a staff member who meet the requirements to perform medication administration.
2. Staff #1 and staff #2 did not have MAT certification.

Plan of Correction: per the administrator; We have required a regional director to be on site at Crestview until our Site Director has completed the MAT class.

Standard #: 8VAC20-780-510-L
Description: Based on observation and interview, the center did not ensure medications, except for those prescriptions designated otherwise by written physician?s order, were kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. Staff #1 stated two emergency medications for child #3 were being kept in the backpack of child #3 per the parent?s request.
2. Two emergency medications were observed in the unlocked back pack of child #3.

Plan of Correction: per the administrator; We have informed parents that medication must be kept in a lock box in the Y office and that they must complete the proper medication request form for the Y to administer medication. We have also trained our temporary Site Director on proper procedures as it relates to medications.
