Standard #: 8VAC20-780-140-A
Description: Based on a review of five (5) children's records and interview, the center did not ensure that one (1) child had a physical examination by or under the direction of a physician:
1. Before the child's attendance; or 2. Within 30 days after the first day of attendance.

Evidence:
1. The record of child #1 (DOE: 06/13/2022) did not contain a physical record.
2. Administration acknowledged that the physical record had not been received.

Plan of Correction: I as the director will not accept any new children without the physical examination filled out by a physician. I called the parent on 9/2/2022 to see if she had kept her appointment for her child examination.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.

Evidence:
1. Chipped and peeling paint was observed in the toddler classroom. A loose wooden board was observed on the playground which contained several protruding rusted nails. A chair with rusted legs was observed on the playground.
2. Administration acknowledged that the areas listed required maintenance.

Plan of Correction: I will have the toddler classroom repainted. I have called a construction company to fix the loose wooden board and replace the rusted nails in the play area. I have removed all chairs with rusted legs from the playground.

Standard #: 8VAC20-780-570-E
Description: Based on observation and interview, the center did not ensure that prepared infant formula was dated as required.

Evidence:
1. The prepared bottles of two children were observed in the infant room refrigerator. The bottles did not contain a date.
2. Administration acknowledged that the bottles were not dated.

Plan of Correction: 9/2/2022 date was added to all prepared infant bottles. I have talked to my infant teacher about the importance of having the name and date on the bottles. This will be checked by the director daily.
