A monitoring inspection was conducted on 08/30/2022. A tour of the facility, interviews with staff and a record review was conducted. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 5staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-280-B Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the preschool classroom there were bleach bottles in an unlocked cabinet underneath the sink and an air freshener in the children's bathroom.
2. In the infant room there were 3 bottles of disinfectant on the counter and another bottle of disinfectant in the unlocked changing table cabinet.Plan of Correction: This was corrected on 8/30/2022. Staff will be retrained on all hazardous substances and chemical procedures. All staff are trained during orientation on safety procedures regarding hazardous substances and chemicals. Staff will be doing weekly compliance safety checks. The safety captain will be doing daily walk throughs. Management will do periodic inspections to make sure policies are being upheld.
Standard #: 8VAC20-780-330-B Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
The center's outside play structures did not have the required resilient surfacing. There was grass and weeds growing above the pea gravel in the fall zones of the rolling climbers and the swings.Plan of Correction: Will be corrected by 10/08/2022. Prior to the visit a request for pea gravel was made and pea gravel was delayed in getting to us. Moving forward a member of management and staff will inspect our physical environment meets safety standards daily. Any deficiencies will be reported to management within 24 hours. Management will also inspect through walk throughs to ensure compliance.
Standard #: 8VAC20-780-350-B-4 Description: Based on observation the center did not ensure the required ratio of 1 staff to every 10 children ages 3 years up to school age was followed.
Evidence:
The Inspector observed, with the director present, 16 children ranging in age from 3-4 years with 1 staff person (Staff #1) in the preschool classroom. The ratio for children 3 years up to school-age is 1 staff for every 10 children.Plan of Correction: Corrected on 08/30/2022 within a minute by immediately placing a teacher into the classroom after a staff member had a bathroom emergency. Staff was retrained on child supervision to ensure all children are within sight and sound and maintaining ratio while in care and what to do in the event of an emergency. All staff are trained during orientation on proper supervision of children prior to being in ratio. A bathroom schedule was established by 9/13 to ensure proper student to classroom ratios at all time. Management will do periodic inspections and ensure a manager is available throughout the day if staff need assistance.
Standard #: 8VAC20-780-400-A Description: Based on observation the center did not ensure that in order to promote the child's
physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide needed help, comfort and support.
Evidence:
From the hallway the inspector heard yelling from the preschool classroom. Through the glass classroom door the inspector observed a cot slide quickly toward the wall close to where a child was standing. With the director present, standing outside the classroom, both heard Staff#1 yelling at a child for not laying on their cot. The Director confirmed that staff #1 was not the children's regular caregiver.Plan of Correction: Will be corrected by 09/16/2022. Staff were retrained on classroom management and child interaction. As of 9/06/22 a new lead teacher has been hired for this room and is out of training 9/12/2022. Additionally 3 new staff members were trained with a master preschool teacher in our district training school to help ensure all teachers as of 9/12/2022 will be in the classrooms they excel the most in to ensure consistency of care. As of 9/12 we will be transferring a member of management from a sister school to fill the role of Associate director. This new position will focus on curriculum and classroom management.
Standard #: 8VAC20-780-440-J Description: Based on observation the center did not ensure there was at least 30 inches of space between service side of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib.
Evidence:
In the infant room there was a baby observed sleeping in a crib. There was a piece of furniture in front of the crib, leaving no space between the crib and piece of furniture, where 30 inches is required.Plan of Correction: The room arrangement will be corrected by 9/16/2022. Staff were retrained on safe sleep and classroom set-up environment on 8/30/2022. Staff will ensure all furniture is put back daily to ensure safe set up. Safety captain will do daily inspections. Management are conducting random classroom observations weekly to ensure compliance.
Standard #: 8VAC20-780-520-B Description: Based on observation the center did not ensure all requirements were met when sunscreen is used.
Evidence:
In the pre-k classroom, there was a bottle of sunscreen on a storage tower in reach of children. The sunscreen was not labeled in any way to identify the owner nor did the center have documentation of written authorization for use. Neither the staff nor the director knew who it belonged to.Plan of Correction: Sunscreen was removed on 8/30/2022. Staff will be retrained on all hazardous substances and chemical procedures. All staff are trained during orientation on safety procedures regarding hazardous substances and chemicals. Staff will be doing weekly compliance safety checks. The safety captain will be doing daily walk throughs. Management will do periodic inspections to make sure policies are being upheld.
