An unannounced monitoring inspection was conducted on-site August 31, 2022. The director was available during the inspection. There were 0 children present as this program is not currently operating. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-150-B Description: Based on a review of children's records on August 31, 2022, the center failed to ensure that immunizations and physicals received shall be signed by a physician, his designee, or an official of the local health department.
Evidence: 1. The record of child #2 contained documentation of a physical exam that was not signed by a physician, his designee, or an official of a local health department.
2. The records of child #2, child #3, and child #4 contained documentation of immunizations that were not signed by a physician, his designee, or an official of a local health department.Plan of Correction: We will correct this by reaching out to the parents of the children with missing signatures and ask them to get the signature for us on these health records. We have given the parents a week to complete this or their child will not be allowed to attend the program until we receive it.
The program will meet this requirement by making sure The Enrollment Coordinator uses the standards to make sure we have all mandated information before enrolling a child into the program and will assure there is a signature or stamp from the doctor?s office on each physical and immunization record before the child is allowed to attend.
This is the responsibility of the Enrollment Coordinator and will be monitored at each site by the Team Leader.
