An unannounced monitoring inspection was conducted today. There were 55 children present with 12 staff.
The children were playing outdoors, resting and working independently .
Five child records and five staff records were reviewed.

Standard #: 22.1-289.035-B-4 Description: Based on the review of five staff records, the facility failed to ensure that all required background checks are completed for all employees.
Evidence:
1. Staff #4 indicated on the sworn statement that they had lived in another state in the past five years. There is no out-of-state central registry clearance on file for staff #4.Plan of Correction: The Head of School reported that the background check request form for staff #4 was submitted on 8-19-22.
Standard #: 8VAC20-780-160-A Description: Based on a review of five staff files, the facility failed to ensure that all staff submit a negative TB screening at the time of employment and prior to coming into contact with children.
Evidence:
1. There are no TB screening results on file for staff #1 & 5. Hire dates: 8-16-22 & 7-11-22.Plan of Correction: The Head of School reported that the staff members will have a TB screening.
