Comments:
An unannounced monitoring inspection was conducted on 9/8/22 from 9:00am to 10:45am with the Director. There were 28 children in care, ranging in age from three- to four-years-old, supervised by four staff. The children were observed rotating through structured activities with the teachers including painting, playing with letters on a light table, making construction paper pumpkins, and playing with playdoh. Six child records and four staff records were reviewed. The Center has five staff with current certification in CPR and First Aid, and seven staff trained in Daily Health Observation Training. Two medications and their authorization forms were reviewed and the center has two staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 7/27/22 and the most recent Health Inspection on file was dated 10/12/21. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/13/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-160-B
Description: Based on review of four staff records, the center did not obtain acceptable forms of documentation of tuberculosis (TB) screening. Evidence: 1. The TB documentation on record for Staff B did not contain a clearance statement or language that the individual does not have any current symptoms of active tuberculosis or is free of tuberculosis in a communicable form.

Plan of Correction: All staff members have been asked to update their TB screening forms and have them signed by a physician or physician representative. Director will follow up to ensure that forms are completed with the appropriate documentation.

Standard #: 8VAC20-780-80-A
Description: Based on review of documentation and interview, the center did not ensure that a written record of daily attendance was maintained that documented the arrival and departure of each child in care as it occurred. Evidence: 1. At approximately 9:05am on 9/8/22 the attendance was requested for the three- to four-year-old class. There were 14 children present but the attendance did not contain documentation of the arrival of any children. 2. At approximately 9:15am on 9/8/22 the attendance was requested for the four- to five-year-old class. There were 14 children present but the attendance did not contain documentation of the arrival of any children.

Plan of Correction: Attendance forms have been updated since last spring to reflect time of arrival and time of dismissal. Staff has been advised via email that they must complete attendance as students are arriving.

Standard #: 8VAC20-780-240-A
Description: Based on review of four staff records, the center did not ensure that the Virginia Department of Education-sponsored orientation course was completed within 90 calendar days of employment. Evidence: The record for Staff A, with a start date of 1/22/22, did not contain documentation of the mandatory training.

Plan of Correction: New staff members have been advised to take the preservice training as soon as possible. Other staff members hired prior to the 10/13/2021 date will also be asked to take the preservice training prior to the end of the school year so that there is a common knowledge base between all staff members.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances, such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A container of disinfecting wipes was observed on a high shelf in the three- to four-year-old classroom. 2. A spray bottle of disinfectant with a ?keep out of reach of children? warning label was observed on a low shelf in the four- to five-year-old classroom as well as several containers of disinfecting wipes in a bin on a high shelf.

Plan of Correction: Because storage constraints do not allow for lockable storage in the classrooms, small storage cabinets with a combination lock have been purchased for use in each classroom.
