A monitoring inspection was initiated on 9/8/2022 and concluded on 9/8/2022. There were 38 children directly supervised by 7 staff members. Children were observed during engaged in group activities and outdoor play time. The physical plant, 3 child records, 5 new staff records, medications, evacuation drills, emergency supplies and policies were inspected. The playground was measured and classrooms were re-measured. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-160-A Description: Based on a review of staff records, documentation of negative tuberculosis screening was not submitted at the time of employment and prior to coming into contact with children.
Evidence: The following staff did not have TB screening at the time of employment and coming into contact with the children, all three staff were observed supervising children this date; Staff # 2, date of employment 8/15/2022, staff # 4, date of employment 8/3/2022 and staff #5, date of employment 8/1/2022.Plan of Correction: Staff will get their TB screening, all staff will have the TB screening prior to hire.
Standard #: 8VAC20-780-70 Description: Based on a review of staff records, documentation that two references were obtained for each staff was not maintained.
Evidence: There was no documentation of two references for the following staff;
Staff # 4, date of hire 8/3/2022 and staff #5, date of hire 6/17/2022.Plan of Correction: I will get the references and file in the staff's record.
Standard #: 8VAC20-780-240-B Description: Based on a review of staff records, staff have not completed the required orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence: The following staff was observed supervising children this date and there was no documentation of orientation training; Staff # 4, date of employment 8/3/2022 and staff #5, date of employment 8/1/2022.Plan of Correction: Staff will receive orientation upon employment.
Standard #: 8VAC20-780-245-J Description: Based on a review of staff records, medications and director interview, there is no staff employed that is trained in the administration of medication.
Evidence: There is one child needing emergency medication for food allergies. There is no one employed that has completed the required training in medication administration of the epi pen medication.Plan of Correction: We will have someone take MAT training immediately.
Standard #: 8VAC20-780-510-I Description: Based on a review of medications, the center has not obtained written authorization from the parent and physician to administer [prescription medication.
Evidence: Child #1 has a prescription epi pen for food allergies, there is no written authorization from the parent or physician to allow the medication to be administered.Plan of Correction: The parent and physician will complete the medication authorization form for weach child's medication.
Standard #: 8VAC20-780-520-C Description: Based on physical plant observation, diaper ointment was not kept inaccessible to the children.
Evidence: The diaper ointments in the toddler class were observed accessible to children in a unlocked cabinet under the changing table.Plan of Correction: The diaper ointments will be moved or a lock placed on the cabinet.
Standard #: 8VAC20-780-570-E Description: Based on observation of the prepared infant formula bottles, bottles were not dated.
Evidence: Two children's prepared bottles were observed in the refrigerator, not dated.Plan of Correction: Bottles will be dated daily.
