Comments:
An unannounced inspection was initiated on 09/01/2022 in response to a complaint that was received by the licensing department on 08/23/2022 regarding allegations of in the areas of supervision and injury reporting. The inspection was initiated on 09/01/2022 and completed on 09/12/2022. On 09/01/2022, the inspector was on site from 8:55 am to 11:30 am. Interviews with staff were completed, observations were made in the center and documentation was reviewed. Additional documentation was reviewed remotely.

The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Description: Based on review of documentation and interview, the center did not ensure that when staff are supervising children, they always ensure their care and protection, and guidance.

Evidence:
1. On 8/22/2022, teacher #1 (date of employment: 01/28/2022) left the infant/toddler classroom for approximately one (1) to two (2) minutes, leaving three (3) children, ranging in age from seven (7) months to 18 months unsupervised. Teacher #1 stated that she heard crying and when she returned, another child was on top of child #1. Child #1 (age 12 months) sustained multiple bite marks and abrasions to the face and lower arms. Child #1 received medical care to treat the injuries on 8/22/2022.
2. The administrator acknowledged that the children in care were not adequately protected when teacher #1 left the room.

Complaint related: Yes
Description: Based on review of documentation and interview, the center did not ensure that children under 10 years of age were always within actual sight and sound supervision of staff.

Evidence:
1. In interview, teacher #1 (date of employment: 01/28/2022) stated that she had four (4) children in care and one (1) child needed a diaper change. She took the one (1) child to the classroom next door and changed his diaper. Staff #1 stated that she ?understood that the children were not in her direct sight? when she left the room to do a diaper change.
2. Administration acknowledged that when staff #1 left the classroom, the children were not in both sight and sound as required.
