Comments:
A complaint inspection was initiated on July 28, 2022 and concluded on August 29, 2022. A complaint was received by the department regarding allegations in the areas of supervision and policies. The inspector reviewed paperwork and interviewed staff over the allegation that Child #1 sustained injuries, the center did not follow policies to ensure transportation of child #1 to and from school, and the center did not follow policies for termination.

The evidence gathered during the investigation did not support the allegation of non-compliance with supervision standards and termination policies. The evidence did support the allegation of noncompliance in providing services for transportation. A violation not related to the complaint was identified during the course of the investigation and can be found on the violation notice.

Description: Based on record review and interviews; it was determined the center failed to ensure that the program?s activities and services were maintained in compliance with their own policies and procedures when they failed to make sure a child arrived to another school in a timely manner on July 22, 2022.
Evidence:
1. The center enrolled a child who required transportation to and from the center to another school. The center agreed to walk the child out of the center, help the child board the school bus, and meet the child off the bus in the afternoon.
2. The staff acknowledged that they missed the bus on July 22, 2022 but did not notify the parent until 2 hours later and then transported the child to school at 11:15.

Plan of Correction: We will improve and enhance our communication processes and train our teachers to avoid incidents like this in future.

Description: Based on record review and interviews it was determined the center did not maintain a complete record for Child #1.
The record for child #1 did not have the parent?s work number. The record also did not maintain a complete physical report for the child. The Physical report maintained in the file did not include the
child?s name or the date of the physical assessment.

Plan of Correction: We take accountability for the error. Going forward, we will make sure to have all forms completed in its entirety per VA licensing standards. Additionally, we will not admit children without all of their full and completed health records. We will audit our files on a regular basis.
