Comments:
An initial inspection was initiated and concluded on Friday, September 9, 2022.The licensing inspector completed an in person inspection at the facility to review compliance of licensing standards including physical plant, programs, special care provisions and emergencies, and special services. There were observations of age appropriate equipment and materials to ensure health and safety for the designated age groups for this program. The LI reviewed all initial application required attachments including background checks, qualifications, and program's policy/procedures were completed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observations and interview, the center did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.


Evidence: The following observations were completed at the center during the inspection on September 9, 2022:
1. In the Infant and Toddler classrooms, there were visible cracked and peeling paint on the walls and window sills.
2. In the Sea Turtle classroom, there is a visible crack in multiple on window siding with a sharp edge accessible to the children in care.
3. On the little playground area, there was a cracked picnic table and ride on tikes with a missing door.
4. On the large playground area, the wooden border was cracked in multiple areas.
5. In the Twos area, the diapering area drawers has multiple areas of peeling paint and a missing knob with a protruding screw.
6. In the Threes classroom, the top layer of the white cabinet was cracked.

Plan of Correction: The center will remove all broken equipment on 09/09/22.
The center will repaint all window sills, drawers, and walls on 09/11/22.
The center will restore knob on change equipment on 09/11/22
The center will replace wooden barriers on play area.
The center will replace shelf.
The center will maintain all sand area and remeasure to ensure adequate supply in fall zones.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Seahorses classroom, the cabinet above the sink was observed unlocked by the inspector. The cabinet contained multiple disinfectant cleaners and sprays with the labels, "keep out of reach of children" "warning" and 'caution".
2. In the School Age classroom, there was a container of disinfectant wipes observed on the desk top unlocked.

Plan of Correction: The center will add a notice on cabinet to remind all staff.
The center director will also remind staff of center's procedures to ensure all cabinets are locked every time.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview, the center did not ensure to obtain all diapering area items as required.

Evidence: 1. In the Infant room, there was not a leakproof covered receptacle for soiled linens found. Interview with staff confirmed a covered receptacle was not available during the inspection.
2. In the infant room, leakproof or plastic lined storage system that is either foot-operated or used in such a way that neither staff members hands nor the soiled diaper touches an exterior surface of the storage system was not found. Interview with staff confirmed the trashcan for disposable diapers was broken during the inspection.

Plan of Correction: The center will purchase a new trash can and leakproof covered receptacle for soiled linens by 09/11/22
