Comments:
An unannounced monitoring inspection was conducted on 09/13/2022. There were 15 children, ages 3 years-4 years, and 4 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in free choice activities, in large motor play and having lunch. A total of 5 children?s records, 5 staff records and 3 medications were reviewed. The inspector arrived to the center at 9:40 am and departed at 11:25 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-510-P
Description: Based on observation and document review, the center failed to ensure that when an authorization for medication for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center.

Evidence: The authorization form to administer Medication 3 was effective from 08/18/2021-08/18/2022. The medication was on-site during the 09/13/2022 inspection.

Plan of Correction: Will monitor the forms constantly and will return the medication this afternoon.
