An unannounced renewal inspection was conducted 9/8/2022 from 10:15amam-2:30pm. There were 73 children directly supervised by 11 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 7 staff records, 7 children?s records, 3 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in a center wide fire drill, playing outside on the playground, listening to story time, playing memory game, playing a virtual piano on an interactive screen, and eating lunch which consisted of english muffin pizza, carrots, pineapple, and milk. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-160-A Description: Based on record review and staff interview, the center did not obtain documentation of a negative tuberculosis (TB) test or screening for all staff at the time of employment that was completed within the last 30 days.
Evidence: Staff #7 (Date of Hire: 6/1/2022) had a TB test on file dated 1/4/2021.Plan of Correction: Staff has documentation of TB test. Dr refuse to issue an interim report of them having one already.
Standard #: 8VAC20-780-70 Description: Based on record review and staff interview, the center did not maintain documentation that all staff have completed orientation training as required by the first day of employment.
Evidence: Staff #7 (Date of Hire: 6/1/22) did not have orientation training documentation on file.Plan of Correction: Staff have been trained. Employee left signed orientation training at home. Director forgot to ask for it.

Evidence: The most recent fire inspection was completed on 4/22/2021.Plan of Correction: All inspections have been scheduled.

Evidence: The most recent health inspection was completed on 9/24/2020.Plan of Correction: Due to COVID inspections are backlogged. Are in schedule to obtain inspection.
Standard #: 8VAC20-780-570-E Description: Based on observation, the center did not ensure that all prepared infant formula bottles were not labeled with the child's name and dated.
Evidence: In the Infant A room, 3 of 11 bottles in the refrigerator were not dated. In the Infant B room, 3 of 7 bottles were not labeled with the child's name or dated.Plan of Correction: Have made parent aware and to ensure labeling of bottles.
