An unannounced renewal inspection was conducted on September 12, 2022 from approximately 9:00 am to 10:00 am. There were 8 children in attendance and a total of 4 staff present. 5 staff records and 4 children?s records were reviewed. All areas of the center were observed including classroom, bathrooms, hallway, and the playground. The children were observed eating breakfast, listening to music, dancing, participating in circle time, and getting ready to go to the playground while the licensing inspector was on the premises.
The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted by each phone.
Violations were cited as a result of this inspection.

Standard #: 22.1-289.058 Description: Based on interview, the center failed to ensure that the center contains at least one carbon monoxide detector. Evidence: When asked, the Director stated that she did not know that they were required to have a carbon monoxide detector. Plan of Correction: Per the center "a carbon monoxide detector was in each classroom on the date of the inspection (09/12/22)."
Standard #: 8VAC20-780-130-A Description: Based on records review, the center failed to obtain documentation that each child received immunizations as required before attendance at the center. Evidence: The record for child #1 (date of enrollment 09/12/22), and child #3 (date of enrollment 09/12/2022) did not contain documentation of immunizations. Plan of Correction: Per the center "Documentation of immunizations for child #1 and child #3 are in the file at the main office."
Standard #: 8VAC20-780-60-A Description: Based on records review, the center failed to document proof of viewing the child?s identity and age in two out of four records. Evidence: The record for child #3 (date of enrollment 09/12/2022) and the record for child #4 (date of enrollment 09/12/2022) did not contain documentation of proof of viewing the child?s identity and age. Plan of Correction: Per the center "#3 corrected on 09/16/22; #4 corrected on 09/16/22; the staff will make sure documentation of the identity and the age of the child is placed in the child's record."
Standard #: 8VAC20-780-550-D Description: Based on records review, and interview, the center failed to practice a monthly evacuation drill. Evidence: Records review documented that the last evacuation drill was 05/17//22. When asked, the Director stated that an evacuation drill was not done in June or July. The program was closed during the month of August. Plan of Correction: Per the center "the evacuation drill was posted on the day of the inspection (09/12/22) for July 2022. The program will ensure staff completes monthly drills."
