Comments:
An unannounced monitoring inspection was conducted from 9:20 - 10:20 am with staff. There were 9 children, age four, present with two staff and a parent volunteer supervising. Children were observed arriving and using the restroom, playing with manipulatives, engaged in circle time and playing in centers. Two staff records, two child records, and three medications, allergy care plans and authorizations were reviewed along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary preferences, and fire and health inspections. The playground was also observed. If you have further questions about this inspection please contact Donna Liberman at 540-359-5244 or Donna.Liberman@doe.virginia.gov.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of two children's records, it was determined that not all records contained all required information. Evidence: the record for child #1, with a diagnosed food allergy, did not contain a written care plan, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: Will ask the parents to obtain an allergy care plan.
