A renewal inspection was conducted on 9/21/2022. There were a total of 95 staff children with 28 staff members present. Children were observed resting, circle time, gross motor skills, and outdoor play. Staff were actively engaged and interactive with the children.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 8 child records and 9 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Based on staff records, the center did not obtain background checks results for employed staff.
Evidence:
1. Staff #3 (date of employment 5/17/2022) did not have child and abuse neglect registry results for PA. The staff member has resided in the preceding within the last five years.Plan of Correction: We will send of paperwork for PA background check results.
Standard #: 8VAC20-780-140-A Description: Based on child records review, the center did not obtain a physical examination before the child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #2 (date of enrollment 4/10/2021) did not have a physical examination in records available for review during the inspection.Plan of Correction: We will requested physical from the parent.
Standard #: 8VAC20-780-160-C Description: Based on staff records, the center did not obtain in staff records at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.
Evidence:
1. Staff #9 tuberculosis screening expired on 9/15/2020.Plan of Correction: Staff will obtain updated TB screening.
Standard #: 8VAC20-780-240-A Description: Based on staff records review, the center did not ensure staff completed the Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Evidence:
1. Staff #3 and #5 did not have Virginia Department of Education-sponsored orientation course completed within 90 calendar days of employment.Plan of Correction: We will have staff complete training by Monday, September 26, 2022.
Standard #: 8VAC20-780-500-B Description: Based on a walk-through, the center did not have an operable leakproof or plastic-lined storage system that is foot-operated or used in such a a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
1. In the Starfish classroom, the disposal foot-operated was not operable.Plan of Correction: We will replace the diaper pail.
